Credentialing Information and Forms

Starting Fall 2022 the Great Plains Conference is using FormVites (part of the database system) for credentialing.  Past credentialing information from 2021-2022 and earlier that was originally entered into the past credentialing system, UMCARES, is now available in Passage UMC (www.passageumc.org) using the same login information used with UMCARES. A help document for FormVites is available in the “Other Forms and Documents” section below.  The easiest way to access a FormVite is through the candidate’s People Portal.

Checklists and forms for all District Committee on Ordained Ministry and Board of Ordained Ministry credentialing are below.  The credentialing candidate is responsible for completing/providing all items included on their credentialing checklist/in their credentialing FormVite unless the checklist of FormVite indicates otherwise.  This includes completion of all forms/documents that may require input from or completion by other persons (such as the District Superintendent report). Unless otherwise indicated, completed documents should only be added to FormVites in PDF format.

Notes:

• Unless otherwise indicated, completed documents should only be added to Passage UMC in PDF format.
• Due to the challenging transition to Passage UMC, there are some modifications in credentialing requirements for 2022-2023. For example, while the SPRC/Leadership Assessment Report will continue to be required for the supervisory process with the District Superintendent, it will not be required for 2022-2023 credentialing.  Some of the modifications may become permanent.
